This Notice of Privacy Practices describes how health information about you may be used and disclosed and how you can access this information. Please review it carefully.

Deb Callahan Therapy is committed to protecting the privacy and confidentiality of your health information. This notice applies to protected health information created or received in the course of providing therapy, supervision-related services where applicable, consultation, billing, and related administrative activities.

Our Responsibilities

Deb Callahan Therapy is required to:

  • Maintain the privacy and security of your protected health information

  • Inform you if a breach occurs that may have compromised the privacy or security of your information

Treatment

Your health information may be used to provide, coordinate, or manage your care. This may include documentation of sessions, treatment planning, consultation, referrals, or communication with other providers when appropriate and authorized.

Payment

Your information may be used to bill and collect payment for services. This may include providing receipts, superbills, or information required for reimbursement when applicable.

Your information may be used for practice operations, such as scheduling, recordkeeping, quality improvement, legal compliance, supervision, consultation, and administrative activities.

  • Sharing information with family members, partners, attorneys, schools, employers, or other third parties, except where permitted or required by law

You may revoke an authorization in writing at any time, except to the extent that action has already been taken based on your authorization.

You may request a paper or electronic copy of this notice at any time.

Access Your Records

You may request to inspect or receive a copy of your health records. Certain limitations may apply as permitted by law.

Request Corrections

You may request that information in your record be corrected if you believe it is inaccurate or incomplete.

Request Confidential Communications

You may request that Deb Callahan Therapy contact you in a specific way or at a specific location.

Request Restrictions

You may request limits on how your information is used or shared. Deb Callahan Therapy is not always required to agree to requested restrictions, except where required by law.

Receive an Accounting of Disclosures

You may request a list of certain disclosures of your health information.

File a Complaint

You have the right to file a complaint if you believe your privacy rights have been violated. You may file a complaint directly with Deb Callahan Therapy or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.

Psychotherapy Notes

Psychotherapy notes, if maintained separately from the clinical record, receive special protection under privacy law. These notes are generally not disclosed without written authorization except in limited circumstances permitted or required by law.

Communication by Email, Phone, Text, or Website Form

Electronic communications, including email, text messages, voicemail, and website contact forms, may not be fully secure. Please avoid including highly sensitive clinical information in website forms or unencrypted email.

Emergencies

This website and routine communication channels are not monitored for emergencies. If you are in crisis, call 988, call 911, or go to your nearest emergency room.

Changes to This Notice

Deb Callahan Therapy reserves the right to update this Notice of Privacy Practices. Any revised notice will apply to information already held as well as information received in the future. The current notice will be available upon request and may be posted on the website.
